If you have difficulty walking or using mobility aids, you may qualify for a Nevada DMV disabled placard or license plate. As a licensed physical therapist in Las Vegas, I am authorized to certify disability forms required by the Nevada Department of Motor Vehicles (DMV).

Who Qualifies?
You must have a permanent disability to qualify for disabled persons license plates.
You may be eligible for a disabled license plate or placard if you:

Cannot walk 200 feet without resting

Use a cane, crutch, walker, wheelchair, or other mobility device

Have a cardiac or pulmonary (lung) condition that limits your function

Are severely limited by arthritis, orthopedic, or neurological conditions

Have a visual disability
-
Use portable oxygen
How It Works

Attend a short evaluation (10–15 minutes)

If you qualify, I will complete and sign DMV Form SP-27

You submit the completed form to the Nevada DMV
